First Approval of Oral Psoriasis Drug: Securing Billions in Milestones and Royalties

Protagonist Therapeutics, Inc. (NASDAQ: PTGX) reported that in its first-quarter 2026 earnings released May 5, it received a $50 million milestone payment (approximately KRW 65 billion) following FDA approval of ICOTYDE, its oral plaque psoriasis therapy. The company said it stands to earn up to an additional $580 million (approximately KRW 750 billion) in milestone payments and 6–10% tiered royalties. In the same filing, Protagonist noted that the new drug application for rusfertide, its polycythemia vera candidate, has been accepted for priority review with a PDUFA action date set for August 2026. On April 28, the company exercised its opt-out right under the U.S. co-development and profit-sharing agreement with Takeda, securing an immediate $200 million payment (approximately KRW 260 billion). Upon approval, it will receive an additional $200 million, a $75 million regulatory milestone (approximately KRW 100 billion), and up to $775 million (approximately KRW 1 trillion) in sales milestones, along with increased royalties of 14–29%. As of March 31, Protagonist held $620.3 million in cash and equivalents (approximately KRW 800 billion), providing a cash runway into at least 2028. Backed by the ICOTYDE milestone and the Takeda payment, the company turned to a net profit of $3.8 million (approximately KRW 5 billion) in Q1, compared with a year-ago loss.

A May 12 SEC filing shows CEO Dinesh Patel exercised expiring stock options and sold 75,000 common shares on the open market—generating hundreds of billions of Korean won in liquidity—while retaining more than 500,000 shares.

In March, the U.S. FDA approved ICOTYDE—an oral IL-23 receptor antagonist co-developed with Johnson & Johnson—as a first-line systemic therapy for moderate-to-severe plaque psoriasis, marking Protagonist’s first commercial product launch. Immediately after approval, several U.S. brokers raised their PTGX price targets to around $100 and maintained positive ratings, underscoring the drug’s commercial potential.

Protagonist Therapeutics is a U.S. biotech company developing oral peptide-based therapies for autoimmune and hematologic disorders.
